The CWCC then recommends the individual for a Professional Staff appointment and recommends a specific delineation of procedural privileges for the applicant. All Professional Staff members are re-credentialed annually.
Professional Staff members who require an appointment at both London Health Sciences Centre and St. Joseph's Health Care London are appointed to one of the following categories within each hospital:
Active
Modified Active
Supportive
Term
Locum Tenens
Honorary
Temporary

Each Department delineates procedural privileges outlining the scope of clinical practice granted to physicians appointed to the Professional Staff.
Privilege categories are recommended to the City-Wide Credentials Committee by the Department Chief and are consistent with the individual's certification, education and/or experience.
As required, Professional Staff members review their appointment and procedural privileges. Requests for changes to the appointment, procedural privileges or both are done through the re-appointment process in April each year.
Applying for an Appointment to LHSC and/or St. Joseph's
As a final step of the selection process for a physician, dentist or mid-wife, the appropriate Department Chief will send a Letter of Offer to the candidate along with a Hospital Appointment Application Package.
The Application Package contains the following documentation to be completed:
Application Form
Declaration and Authorization for Release of Information
Request for three letters of reference
Institutional request for certificate of Professional Conduct and Release
CPSO Consent for release of Information
City-wide Immunization Health Screen Form
Privacy and Confidentiality Agreement
Department Chief Support
Documentation to include with the Application Package includes:
Verification of current registration or license to practice in Ontario
Verification report of current malpractice insurance information for Ontario (i.e., CMPA, AOM)
Current Curriculum Vitae
Copy of Medical, Dental or Midwifery school certificate
Copy of Fellowship Certification
Completed Application Packages need to be submitted to the Department Chief 10 weeks prior to the anticipated start date of the new Professional Staff member.
All Professional Staff appointments are for the period July 1 to June 30th. In March of each year, Medial Affairs sends an e-mail to all Professional Staff members to provide information and a link to access an on-line re-appointment website.
Professional Staff members are given a unique i.d. and password to electronically open and complete their re-appointment form and submit it to Medical Affairs.
Medical Affairs manages the process to validate each appointment obtain support from the Department Chief, obtain approvals from the City-wide Credentials Committee, the Medical Advisory Committee and the Boards of Directors. If any changes are required, Professional Staff will be notified by the Department Chief.
Professional Staff are notified by letter when all approvals are complete.
