All new appointees to the Active Staff are required to complete a 12-month probationary period. The Department Chief or an appointed supervisor works with each new Active member and ensures that at 6 and 12 months, the performance of the individual is evaluated.
The evaluations are recorded using either the Preliminary Probationary Evaluation Form for Professional Staff or the Final Probationary Evaluation Form for Professional Staff.
Members who are appointed to the Term Staff may be required to do a probationary period at the discretion of the Department Chief. Satisfactory Reviews If the performance of the Probationary Professional Staff member is "satisfactory" on the 6-month and 12-month review, the City-wide Credentials Committee (CWCC) will make a recommendation to the Medical Advisory Committee (MAC) who will subsequently make a recommendation to the Board to renew the active appointment without probation. Unsatisfactory Reviews If, at either the 6-month or 12-month review, the performance of the Probationary Professional Staff member is "unsatisfactory", the following actions are initiated:
The supervisor arranges a meeting for him/herself, the Department Chief and the Professional Staff member. At this meeting, the supervisor advises the member of the concerns about his/her performance.
The supervisor documents the information and concerns for the Department Chief and the City-wide Credentials Committee.
The Credentials Committee reviews the unsatisfactory evaluation and, if warranted, makes a recommendation to the MAC who will subsequently make a recommendation to the Board to end the appointment.
If the appointment is to continue with re-evaluation in six months, the supervisor develops a "remediation" plan that includes regular follow-up meetings. At these meetings, the supervisor ensures that the documented concerns are addressed. The remediation plan is shared with the City-wide Credentials Committee
