When admitting a Patient, you will need to provide the:
Patient's Name
Patient Idenitification Number (LHSC) or Medical Record Number (SJHC)
Admitting Physician
Diagnosis
Infectious status or Isolation Requirements (e.g. MRSA, VRE, TB)
Special needs such as ECG Monitoring or Telemetry
Estimated length of stay (SJHC)
IMPORTANT: When accepting a transfer patient for admission, please remember to call patient registration (LHSC) or admitting (SJHC) BEFORE accepting the patient to determine if a bed is available.
